I am broadly interested in the older patient’s psychological reaction and adaption to medical illness -- at the intersection of clinical implementation, biomedical ethics, and healthcare policy.
Clinical Innovation for Mental Health Treatment in Multi-Morbid Geriatric Patients. My recent work focuses on cancer survivors. With colleagues I have investigated the unique role of combat PTSD in moderating depression and anxiety after cancer. I have used this work to implement a mental health treatment program for cancer survivors and an electronic medical record survivorship "wellness plan" to link providers in post-treatment care.

Clinical Innovation in Capacity Assessment. My recent work focuses on public guardianship. In talking to older adults, I became interested in how patients make decisions about their care, and how providers view their "capacity" to make those decisions, particularly if there is mild cognitive impairment. With the goal of improving our understanding of how vulnerable patients make medical decisions, my research has investigated how the patients' values can be brought into the consideration of decisional processes. I have translated these findings to practice by editing a series of American Psychological Association (APA)- American Bar Association (ABA) practice handbooks to improve capacity determination in light of values by lawyers, judges, and psychologists – disseminated to more than 100,000 clinical and legal professionals in the US and internationally. I presented my scientific findings in legislative testimony for guardianship law reform. I have consulted to numerous professional groups with broad national impact including the Investment Protection Trust; the Financial Industry Regulatory Authority; Association for the Accreditation of Human Research Protection Programs; and Veterans Benefits Administration Insurance Service. Recent papers synthesizing my perspective on capacity assessment have appeared in the Journal of the American Medical Association and the American Psychologist. My work in this area has been awarded local and national honors including The APA Award for Advancement of Psychology and Aging.

Teaching and Education. I am a primary clinical and research supervisor to geropsychology interns and fellows (since 1997). My achievements in this area were recognized by APA Society for Clinical Geropsychology Distinguished Mentorship Award. In addition to published scholarship in the area of geropsychology training, I am currently completing funded work (Council of Professional Geropsychology Training Programs; APA Board of Educational Affairs) to study and advance geropsychology training in clinical, research, and teaching competencies.

In addition to these activities, I am active in serving the VA, HMS, National geropsychology, and local Dorchester communities. I am the past chair of APA’s Committee on Aging – the major standing committee on aging within the APA governance structure. My service to the Dorchester community was recognized by an HMS Dean’s Award for Community Service.

In summary, my clinical innovation work in geropsychology has integrated clinical research, clinical translation and implementation, and clinical training and scientific mentorship, with the ultimate goal of advancing psychological assessment and treatment of older adults -- with a focus on the patients' values and perspectives. Across these activities, my goals are to support patient centered care; improve patient-clinician communication; and enhance policies and processes of care within healthcare and health law.
