BIDMC's PACT (Post-Acute Care Transitions) nurse/pharmacist program to prevent avoidable 30-day readmissions began in 2012 through funding as a Center for Medicare and Medicaid Innovation awardee. We are now looking to explore new models of deployment, patient and provider interaction to expand scale and improve efficacy of this transitional care program designed to optimize outcomes after hospital discharge. An interested student would perform observations, review charts, and collect staff feedback as part of an operational design process to inform future staffing models for this program.

Recent configuration of the Medicine service into Fully Integrated Regionalized Microteams (FIRMs) has created a platform for improved teamwork amongst physicians, nurses, and case managers, and ancillary staff. We are applying the Relational Coordination improvement framework pioneered by Professor Jody Gittell (currently at the Heller School of Social Policy and Management at Brandeis University) to identify focused areas for improved team function, and design interventions to achieve those improvements. An interested student could assist in observations, implementation/design of improvement interventions, and data analysis of Relational Coordination survey results.
