Our goal is to improve the immunization rates of the adolescent patients in our primary care practice. We have tracked and worked on immunization rate improvements in our practice for children under two years of age for more then 20 years. Our hospitals adolescent practice has also started to work in this area, but we have so far done little or no work here. The baseline question will be to analyze our practices immunization rates compared to national and state benchmarks that have been previously published (and are updated yearly) by the CDC. We then will plan to put into place interventions to improve those rates with a goal to measure the immunization rates repeatedly as part of a quality improvement project, offering students both short term and long term projects that they could work on.
