I became interested in risk reduction for HIV-serodiscordant couples who choose to conceive while working as an internal medicine resident in Rwanda (2006) and as a medical consultant in South Africa (2008). I met many HIV-infected men and women who were eager to conceive children, but did not know how to do this without infecting his/her partner. A young, fertile population in HIV-endemic regions demands HIV-prevention strategies that address couples’ reproductive goals. Antiretrovirals as prevention (antiretroviral treatment for the positive partner and pre-exposure prophylaxis (PrEP) for the uninfected partner) are particularly exciting options for periconception risk reduction. Using behavioral science to understand sexual and reproductive behavior in HIV-endemic settings, I plan to develop and implement feasible, acceptable safer conception interventions for HIV-affected couples.

In 2009, I published a harm-reduction approach on minimizing sexual HIV transmission while respecting couples’ reproductive choices (AIDS and Behavior). In 2011, I received a K23 award (NIMH, 1° mentor Bangsberg) to investigate fertility desire, pregnancy incidence, and ARV adherence in a cohort of women on ART and among women enrolled in the Partners PrEP trial; and develop and pilot a reproductive counseling intervention to minimize HIV transmission risk among serodiscordant couples who choose to conceive. I also secured R03 funding (NICHD) to carry out a mixed-methods study to explore provider-patient discussion of fertility plans and safer conception or contraception options in KwaZulu-Natal, South Africa. Through mixed-methods work in South Africa, I found that people living with HIV prioritize conception over HIV-transmission risks and are eager for safer conception advice, that men make most conception decisions, that most men and women don’t know their partner’s HIV status, and that healthcare workers lack safer conception knowledge (AIDS and Behav 2011, Repro Health Matters 2012, Infect Dis Obst & Gyn 2012, BMC Public Health 2014, JAIDS in press.) This led to R34-funding to develop an intervention for HIV-infected men who choose to conceive with at-risk partners. I pursue parallel work in Uganda where a total fertility rate of 6.1 children per woman makes safer conception essential (PLoS ONE 2013, AIDS 2014).

I also spend 20% of my time as an infectious disease attending at MGH on teaching services that include fellows, residents, and medical students.
