    Transforming Residency Training for the Future of Healthcare Delivery: Implementing a Population Health Strategy in a Resident Continuity Clinic
    Role Description: To transform resident continuity clinic into a population health model of care delivery. We aim to make this major structural and change to improve the educational experience for our residents, and the clinical care of their patients. We hope to provide a model for residency programs across the country.
