This project will assess health disparities in blood pressure control at a med-peds primary care clinic site at Brigham and Women's Hospital (Brigham Family Care Associates in Brookline). We will also be developing an intervention to bring patients to blood pressure goal, including hypertension education materials, HTN focused follow up visits, and home blood pressure monitoring resources. The student will help with: 1) Data analysis for a hypertension clinic dataset that will compare clinic patients who are/aren't at blood pressure goal. 2) Develop hypertension educational materials for distribution at the clinic 3) Help develop a plan/content for hypertension follow up visits 4) Attend meetings (approximately every other month) with the Brigham Med-Peds residency program project organizers and/or the Brigham Health Equity Innovation Program to discuss project and other health equity initiatives happening at BWH. This project would be a great fit for students interested in either: 1) Social justice and health equity 2) Internal Medicine or Medicine - Pediatrics 3) Primary Care
