The percentage of women who undergo mastectomy and reconstruction in the United States is increasing and as of 2016, over 40% of women had reconstruction. Given the prevalent nature of breast cancer, increasing rates of reconstruction, and preference-sensitive nature of reconstructive choices, there has been increasing emphasis on improving quality of life (QOL) outcomes. At the same time, as healthcare systems seek to provide optimal patient care in the face of increasing budgetary scrutiny, an understanding of the true cost of breast reconstruction as it relates to QOL is required. Prepectoral implant placement has made a resurgence in popularity in the United States largely due to improved mastectomy techniques, human acellular dermal matrix, and patient preference. This technique is relatively novel and robust evidence regarding clinical effectiveness and costs is lacking. This project seeks to overcome these limitations through a rigorous, prospective study comparing patient reported outcomes (PROs), clinical outcomes, and cost-effectiveness of prepectoral and subpectoral implant reconstruction after mastectomy. The role for a student would include: orienting potential subjects to this trial, participating in patient enrollment and thus getting valuable patient-facing experience, ensuring that patients meet the various study-related benchmarks, data validation and analysis. The aims of this study will be to prospectively evaluate PROs in women undergoing two-stage IBR in the prepectoral or subpectoral planes using the BREAST-Q Satisfaction with Breasts scale. In addition, we will prospectively evaluate costing data using Time Driven Activity Based Costing in these two cohorts. We will then combine these data to perform a robust cost effectiveness analysis to ascertain optimal value. We hypothesize that the additional costs incurred by prepectoral reconstruction will be justified by increases in PROs when two-staged, prepectoral reconstruction is compared with subpectoral reconstruction. This will be the first study which prospectively evaluates value by comparing PROs while accurately measuring costs.

The student researcher will join one of the main areas of investigation in our group which includes (1) Improving cost-effectiveness and value in surgical care, (2) Evaluation of novel technologies in surgery, (3) Implementation of patient reported outcome measures (PROMs) in surgery, and (4) Improving clinical outcomes in breast reconstruction. Students will be given the opportunity to familiarize themselves with the topic as well as develop their skills in critical review of literature, development of a research plan, interfacing with colleagues from the Business School as well as the School of Public Health, and publish individual research. Our research team fosters a collaborative environment in which students have the opportunity to work with members of other research groups (PROVE center and CSPH) at our institution. Research meetings are held weekly where students can share the progress of their research and hear updates on projects by fellow researchers.
