Tolerance of kidney allografts has been achieved in non-human primates (NHPs) and in humans using a combination of nonmyeloablative conditioning and donor bone marrow transplantation (DMBT) that results in transient donor hematopoietic chimerism. However, this procedure fails to induce tolerance in NHP recipients of heart allografts. It is well known that some organs, such as kidney and liver, are immune-privileged and tolerance-prone, while others, such as heart and lung, are tolerance-resistant. In earlier studies using MHC inbred miniature swine, we took advantage of the tolerogenicity of kidney allografts and, through donor kidney cotransplantation, achieved long-term tolerance of heart allografts which, if transplanted alone, are rejected acutely. Mechanistic studies in swine showed that kidney allografts were able to confer tolerance upon co-transplanted hearts by promoting the expansion of regulatory T cells (Tregs).

Recently, by combining our mixed-chimerism protocol that induces transient donor chimerism with donor kidney cotransplantation, we have succeeded in achieving long-term tolerance of MHC-mismatched heart allografts. Importantly, every recipient that successfully completed its tolerance protocol achieved indefinite allograft survival without evidence of cardiac allograft vasculopathy (CAV). These results are the first to demonstrate tolerance induction in NHP heart allograft recipients and the first to show that long term unresponsiveness to a tolerant-resistant organ can be achieved without durable donor chimerism.

Given our findings in swine and NHPs demonstrating that the tolerogenicity of kidney allografts is based upon regulatory mechanisms, we hypothesize that amplifying the contributions of host Tregs while limiting the deleterious activity of memory T cells (Tmems) in NHP heart allograft recipients undergoing mixed chimerism conditioning will achieve long-term tolerance without the need for kidney cotransplantation. To test this hypothesis, we will determine whether two clinically relevant strategies known to suppress memory T cell immunity while enhancing the in vivo contributions of Tregs will successfully substitute for the kidney cotransplantation and induce tolerance of isolated heart allograft. The successful completion of these studies could yield tolerance protocols ready for immediate evaluation in human heart-alone recipients.
