Patient Satisfaction: We are looking for 1 student to select, design, and implement methods to collect patient feedback from an ethnically & linguistically diverse patient population regarding their experience receiving care at a student-faculty collaborative practice providing integrated primary care, mental health, & social services. This student will propose improvements to the clinic model based on analysis of their data. This project will be mentored by Marya J. Cohen, MD, MPH. She is a primary care physician at the MGH-Chelsea Health Center, faculty preceptor and medical director at the Crimson Care Collaborative at MGH-Chelsea, and the Principal Investigator for the HMS Center for Primary Carefs Academic Innovations Collaborative at MGH-Chelsea. Student projects at CCC-Chelsea have been featured in conferences for the Society of General Internal Medicine and the Society of Student-Run Free Clinics. Students in need of funding for this project should contact the Center for Primary Care. Speaking Spanish would likely be helpful but is not required. Time commitment is ~ 4 hours/week but will vary according to the student's schedule and demands of the project. Duration of project is also flexible but can start as early as October 2012.

Post-Incarceration: CCC-Chelsea has a special focus on addressing the social, psychological, and structural issues impacting people re-entering the community following incarceration. 3-4 students are invited to research questions related but not limited to mental health, substance abuse, and other medical and social needs of a post-incarceration population. These students will also have the opportunity to serve on CCC-Chelsea’s post-incarceration outreach team, whose activities will include conducting a needs assessment in partnership with our patients, working on a County Task Force for post-incarcerated women with dual mental illness and substance abuse diagnoses, connecting with discharge planning services at area prisons, collaborating with other community health centers serving a post-incarceration population, and participating in additional advocacy and educational initiatives as determined by the group.

Integrated Primary Care in a Community-based Setting: CCC-Chelsea is a student-faculty collaborative practice providing integrated medical, mental health, and social services to frequent utilizers of urgent care who lack primary care providers as well as people re-entering the community following incarceration. Through the on-going efforts of our research team, we have learned that 76% of our patients have either MassHealth or Health Safety Net as their insurance, 34% of our patients earn less than $15,000 annually, and 74% of our patients demonstrated at least one social service need. Students interested in conducting research regarding health disparities, health care delivery, quality improvement, medical education, or other topics of their choosing in this practice environment are invited to join our clinic staff to design and implement their scholarly project. Patient Engagement: We are looking for 1 student to measure the impact of interventions such as appointment reminders and social service follow-up aimed at improving patient engagement in primary care. This student will examine outcomes such as no-show rate, completion of community referrals, and additional metrics of success as defined by the student investigator. This student will be responsible for incorporating lessons learned from these interventions in the revision of existing practices and development of new techniques.
