Tobacco use is the leading preventable cause of death in the US and worldwide, but health care systems are not designed to address this chronic condition in a coordinated way. Such as system of care would include strategies for population management as well as interventions in hospital and outpatient settings. The MGH Tobacco Research and Treatment Center, in collaboration with Partners HealthCare System's Tobacco Task Force, is building the components of a coordinated system of tobacco treatment. Ongoing pilot projects included (1) testing an extended care model to sustain tobacco treatment after hospital discharge. (2) a population management strategy to offer treatment directly to smokers identified from medical records, and (3) a novel intervention in the electronic health record to link smokers seen by primary care clinicians to a central tobacco coordinator who can link to treatment resources in the community and co-manage a smoker's care based in the primary care practice. Each project has opportunities for HMS or HSDM students to participate in care redesign, evaluation design, data collection and analysis, and preparation of reports.
